To make an appointment:

Please print off the letter we have prepared for your medical advisor. Which can be found here. Your consultant will forward your referral to the Cancer Specialist/Patient Referral Team either by fax/post.

If you would like to suggest to your medical advisor which of our consultants you would like to see:-

• Please go to Which Consultant to find the names of the LOC Consultants specialising in your cancer.

If you are a patient requiring assistance with an appointment please choose from the following options.

• Please complete the Appointment Form – one of our nurse advisors will return your call within 24 hours or on Monday if you have submitted the form over the weekend or;
  
• Please call the Patient Referral Team between Monday – Friday 9.30 – 5pm.
